| Medical Staff Membership Information |

Medical Staff - An application packet for appointment to the Medical Staff
and/or for clinical privileges will be provided to an inquirer who meets the
following eligibility criteria:

Graduate of an approved medical, osteopathic, dental or podiatry school.

Licensed to practice medicine, dentistry, or podiatry in the state of Texas.

Certified by or has completed the training requirements for Specialty Board
certification.

Allied Health - An application packet for Allied Health Professional status and
clinical privileges will be provided to an Allied Health inquirer who meets the
minimum eligibility criteria for the category for which they are applying.

All practitioners must have primary residency and primary medical practice in
Lubbock County, Texas.

|
A copy of a current Texas licensure must accompany all requests.
