Clinical Rotation Students

Thank you for choosing Covenant Health for your clinical rotation experience! We look forward to having you. Please provide the documentation below to complete your student onboarding.

*Student’s school/university must have an affiliation agreement with Covenant Health before participating in clinical rotations at any Covenant facility.

*Students are responsible for procuring a preceptor. Covenant Health does not match the student with a provider. If you do not have a preceptor, you can go to https://www.providence.org/doctors, enter the specialty that you are interested in, click on a provider, and call the office number to ask if that provider is willing to supervise you while you are doing rotations at Covenant.

To streamline your onboarding, the information below must be received a minimum of two weeks prior to your start date for processing.

  1. Clinical Rotation Application (Includes acknowledgment of attestations.)
  2. Download and complete Signature Forms (Supervising Professional Agreement must be signed by both you and your preceptor(s) with start-end dates that you will be rotating. Attestation of Immunizations and Background Check must be signed by your school advisor)
  3. Certificate of completion for watching the online clinical rotation orientation and taking a short quiz.
  4. Photo (Jpg format) for your student badge
  5. Watch the Red Rules Video at https://vimeo.com/396009520 using the password RED. (all caps)

Email (2-4) to Rochelle Bell at [email protected]. After the application, signature forms, certificate of completion, and photo have been received, your badge will be ordered, and you will be notified of when/where to pick it up. *Badge pickup is not available on Thursdays.

Complete OR Scrub Training (if applicable)

  • If you are rotating in the OR, Cath Lab or L&D, you will need to go through a face-to-face scrub assessment. OR contact information will be emailed to you. Please let me know at least one week prior to needing this training.
