Clinical Rotation Students

Thank you for choosing Covenant Health for your clinical rotation experience. Below are the steps required to complete the onboarding process.

*Student’s school/university must have an affiliation agreement with Covenant Health before participating in clinical rotations at any Covenant facility.

*Students are responsible for procuring their own preceptor. Covenant Health does not match the student with a provider.

1. Complete the Covenant Health Clinical Rotation Application and Attestations

The completed application and other required documents listed must be received a minimum of two weeks prior to start date for processing.

2. Download, complete, and return Signature Forms and a photo (jpg format) to bradleyrd@covhs.org. (Supervising Professional Agreement must be signed by both you and your preceptor(s) with start/end dates. Attestation of Immunizations and Background Check must be signed by your school advisor)

3. Watch the Covenant Health online clinical rotation orientation. https://covenanthealth.inreachce.com/Details/Information/2f21698a-c588-4733-bd80-cbe42df0a4d0 Dietetic Interns/EMS Students will have a different orientation.

  • Under Guest, click on Sign In, then create your account.
  • Select the Clinical Rotation Student Orientation which is free.
  • Download handouts and watch the training video.
  • At the completion of the training video, you will be prompted to take a 12-question quiz over the information presented in the video. You must score 80% to pass and have two chances to pass.
  • Once you receive your certificate of completion on the main page, please forward same to bradleyrd@covhs.org.

5. Pick up your Covenant badge ***PLEASE NOTE THAT BADGE PICK-UP WILL NOT BE AVAILABLE ON THURSDAYS. ***

  • When the completed application, photo, and signature forms have been received, your badge will be ordered, and you will be notified when/where to pick it up,

6. Epic training modules (When applicable, login information will be emailed to you after all required documents have been received.)

7. Complete OR Scrub Training (if applicable)

  • If you are rotating in the OR, Cath Lab or L&D, you will need to go through a face-to-face scrub assessment. OR contact information will be emailed to you.
