Medical Staff Membership
Thank you for your interest in Covenant Health. We have included our Threshold
Eligibility Criteria below that must be met in order to apply for initial
appointment at our facilities. Additional information can be found in
our Credentials Policy under 2.A.1 Threshold Eligibility Criteria.

All applicants must:
- Have a current, unrestricted license to practice in Texas and/or New Mexico
depending on coverage location.
- Never have had a license to practice revoked or suspended by any state
licensing agency.
- Provide appropriate call coverage.
- Have current, valid professional liability insurance.
- Never have been convicted, or entered a plea of guilty or not contest to
payer fraud.
- Never have been excluded, precluded or debarred from participation in Medicare,
Medicaid or state/government health care programs.
- Never have had Medical Staff appointment nor privileges denied, revoked
or terminated by any facility or health plan for reasons related to clinical
competence or professional conduct.
- Never have resigned nor relinquished medical staff appointment or privileges
during an investigation or in exchange for not conducting an investigation.
- Never have been convicted of, or entered a plea of guilty or no contest,
to any felony.
- Never have been convicted of, or entered a plea of guilty or no contest
to any misdemeanor relating to controlled substances, illegal drugs, insurance
or health care fraud or abuse, child abuse, elder abuse or violence.
- Demonstrate recent clinical activity in their primary area of practice
during the last two years.
- Meet eligibility requirements that are applicable to the clinical privileges
being sought.
- Document compliance with all applicable training and educational protocols.
- Document compliance with any health screening requirements.

Have successfully completed:
- a residency or fellowship training program approved by the ACGME or AOA
in the specialty in which the applicant seeks clinical privileges; or
- a dental or oral and maxillofacial surgery training program accredited
by the ADA; or
- a podiatric surgical residency program accredited by the American Podiatric
Medical Association.

Satisfy the following requirements pertaining to board certification:
- Have obtained board certification at some point in their career and not
lost it due to misconduct (in that case, would need current board certification); or
- Become board certified within 5 years of initial appointment and during
that five years, obtain all TMB required CMEs in their area of specialty; and
- Board certification must be obtained through an organization approved by
the ABMS, the AOA, the ABOMS, the ADA or the ABPM.
