Observer is defined as one enrolled in a pre-medical higher education program that is 18+ years of age, but not yet to the level of patient care. An observer will shadow a Covenant Health medical professional with no hands-on learning and should never at any time participate in care delivered to the patient. This process is not offered for those completing the credentialing process.
Exceptions:

Students 16+ years of age may be considered when providing a letter of recommendation by a school official on your school's letterhead.

Visiting Physicians, Allied Health, and Nursing will be considered on a case-by-case basis.
**OBSERVER MUST PROCURE THEIR OWN SUPERVISOR**
Follow the instructions below to complete the onboarding process: All information should be submitted a minimum of 4 weeks prior to observer start date for processing.

Complete the Physician/Professional Observer Application (Includes acknowledgment of attestations)

Download and complete Signature Forms

Notice of and Consent to Background Investigation form to be filled out and signed in two places. (Allow 2-4 weeks for background check clearance and application follow up.)

Supervising Professional Agreement must be signed by both the observer and the supervising provider(s) with start-end dates that you will be shadowing.

Student Health Requirements form (New form effective 11/25/24)

Observer submit proof of immunizations to your supervisor (the provider you will be shadowing). Form should be signed/dated by the observer on the signature line

Supervising Provider (the provider you will be shadowing) should confirm immunizations are up to date, check off, sign/date on the administrator signature line. *If your supervisor would like me to keep immunizations on file, please email me a copy and I will house them with your application.

Covid and Flu Declination forms can be filled out if you have not received and do not plan to get the most recent vaccinations.

Photo (Jpg format) for your observer badge
Email (2-4) to Rochelle Bell at [email protected]. After the application,
signature forms, certificate of completion and photo have been received,
your badge will be ordered, and you will be notified when/where to pick it up.
*Badge pick-up is not available on Thursdays at Lubbock facilities. A $10 fee will be charged for lost badges that need to be re-printed. Cash Only.
